Evolving Papilla Reconstruction: Interdisciplinary Decision-Making, Novel Periodontal Techniques, and Esthetic Implant Strategies

The reconstruction of the interdental papilla is one of the most demanding challenges in contemporary dentistry. Open embrasures—commonly known as “black triangles”—often persist despite restorative, orthodontic, or surgical interventions due to segmented, discipline-specific approaches. Predictable improvement requires accurate diagnosis of the underlying cause and the application of coordinated, interdisciplinary treatment strategies.This presentation will explore an interdisciplinary diagnostic and decision-making framework for managing open embrasures, with particular emphasis on periodontal surgical interventions. I will introduce a range of surgical approaches, focusing on SAPRe (Supra-Alveolar Periodontal Reconstruction) and DEPP (Double-Sided Entire Papilla Preservation) as representative techniques that allow for effective hard and soft tissue management without direct papillary incision. These approaches will be discussed alongside other established methods to highlight their indications, rationale, and clinical outcomes. Additionally, the application of these principles to implant therapy will be discussed—particularly strategies for papilla reconstruction around existing implants, and implant placement protocols that aim to achieve an ideal papilla through biologically and esthetically guided hard and soft tissue management.
