Black Triangle Management: Advanced Soft Tissue Grafting for Esthetic Zone Rehabilitation

The reconstruction of lost interdental papillae remains one of the most demanding challenges in dentistry, particularly in the highly visible esthetic zone. The presence of “black triangles” in the anterior maxilla can significantly compromise esthetic outcomes and patient satisfaction. This presentation introduces a minimally invasive technique to address papilla loss around both teeth and dental implants, utilizing a customized subepithelial connective tissue graft in combination with a three-dimensional (3D) tunneling approach. This method is specifically designed to preserve the vascular supply while allowing precise positioning and stabilization of the graft, with the goal of augmenting soft tissue volume and promoting papilla regeneration. Clinical outcomes demonstrated improved soft tissue contours, both complete and partial papilla reconstruction, and enhanced esthetic integration around teeth and implants. This technique highlights the importance of individualized soft tissue management and meticulous interdisciplinary planning in addressing complex esthetic challenges.
